Summary:
• The contractor will support the by coordinating meetings, facilitating implementation plans, monitoring progress, assisting county health departments, and ensuring compliance with data security standards
The Contractor must possess the following minimum qualifications and experience: 5.1. A minimum of one year of public health practice. 5.2. A minimum of two years with strategic planning, meeting facilitation, partner engagement, and handling multiple projects. 5.3. The assigned staff member must be able to work 40 hours per week with office space provided at the
RESPONSIBILITIES:
6.1. SERVICE TASKS: Contractor will perform the following tasks in the time and manner specified: 6.1.1. Schedule recurring meetings, (at a minimum biweekly) with PAW co-chairs to coordinate meetings and other activities including implementation plans development and progress tracking. 6.1.1.1. The implementation plan will be used to track and monitor the activities and progress of the PAW that contribute to the goals , strategies and objectives outlined in the State Health Improvement Plan. • These activities are usually already being implemented by Division of Public Health Statistics and Performance Management • Bureau of Community Health Assessment Formal Scope of Work Public Health Consultant for State Health Improvement Plan(SHIP) SOW26-077 the partner organizations and are being introduced to the SHIP via the PAW implementation plan for inclusion. • Department staff will provide technical assistance and guidance in the development. • The Department will assist in the development of the PAW specific implementation plans and progress reports. • The Department will also monitor progress of the SHIP goals, strategies, objectives in the chosen system for monitoring performance measures for the Department and provide that information to the PAWs as a resource. 6.1.2. Schedule PAW meetings to be held quarterly, or more frequently as needed. 6.1.3. Work with PAW co-chairs in the facilitation of implementation plan development with collaboration from all members of the PAW. 6.1.4. Provide consultation to PAW co-chairs on the tracking and monitoring of activities included in implementation plans as directed by the Department. 6.1.5. Work with PAW co-chairs to develop meeting agendas, facilitate meetings and take meeting notes as needed. 6.1.6. Send out quarterly data reminders to PAW co-chairs, check data system for completeness, follows up with PAW co-chairs to facilitate complete quarterly reporting on the PAW objectives as directed by the Department. 6.1.7. Assist PAW co-chairs with the SHIP Annual Progress Reports, identifying recommendations for revisions to the goals and objectives, and revisions to the annual implementation plans as needed. Submit revisions to the Department Contract Manager for completeness. 6.1.8. Assist PAW co-chairs in preparing presentations for the Full PAW meetings, including all rostered members of the priority area workgroup and the SHIP Steering Committee meetings as directed by the Department. 6.1.9. Schedule weekly meetings with the SHIP lead within the Division of PHSPM to discuss updates on progress, barriers encountered and develop action plans for the upcoming week as directed by the Department. 6.1.10. Document best practices from PAWs and communicate to the SHIP lead in the Division of PHSPM as directed by the Department. 6.1.11. Ensure PAW member lists are up to date and kept current. Store in a central location as directed by the Department. 6.1.12. Implement a tracking system to record the activity of each PAW to help ensure a minimum level of activity and functions as directed by the Department. 6.1.13. Assist county health departments with their health assessment and health improvement planning processes to include: 6.1.13.1. Review of Community Health Assessments and Health Improvement Plans as assigned. 6.1.13.2. Communicate (electronically or in person) with county health department staff to provide technical assistance with their Community Health Assessment and Health Improvement Plans as directed by the Department. 6.1.13.4. DATA SECURITY AND CONFIDENTIALITY TASK: The Contractor, its employees, subcontractors, and agents must comply at all times with all Department data security procedures and policies in the performance of this scope of work as specified in the Data Security and Confidentiality document attached to the purchase order.
